Registration Fees

$ 190.00 (CDN)– on or before February 17, 2012
$ 205.00 – (CDN) after February 17, 2012
Student Rate, with valid student card – $ 115.00 CDN
Please note that student rate is only applicable to full-time student or resident participants.
A conference syllabus is included in the Registration fee.

Registrations

A late registration fee is in effect. You will receive a registration confirmation at the fax, address or email address you have provided when your registration and payment is received. If you have not received a conformation within 5 days, your registration was not received in our office. Receipts and CME credits will be included with your syllabus package at the door.

Cancellation & Refund Policy

The sponsors reserve the right to cancel if registration is insufficient. Registrants would receive notification by phone or in writing with a full refund to follow. Nova Clinical Services Inc. would not be responsible for other costs including (but not limited to) airline or hotel penalties.

Should registrants wish to withdraw, notice must be received no later than February 17, 2012. Refunds will be made less a $ 25.00 CDN handling charge. If withdrawing after February 17, there will be a $50.00 CDN handling fee. There will be no refunds on the day of the conference or after the conference.
